Summer Camp Forms

We want to make your summer camp preparation a little easier. We've added some of the forms that we normally send in our camp confirmation packets online.

 


General Information

Who: All students, elementary through college, may attend Starmakers Dance and Performance Camps.

What: You should plan to bring two bed linens, pillow, blanket, and towels (Southwestern Line Camp only).  Bring your CD player and batteries, plus spending money for snacks and shopping in the Starmakers camp store (all camps). 

When: In most cases, Hotel Camp registration will be from 8:00 am to 9:00 am.   Hotel Camp will begin at 9:00 am on the first day and end at 7:00 pm on the 3rd day. Southwestern camp will register from 9:00 am to 11:00 am with camp beginning at noon on the first day and ending at noon on the fourth day.Any variations from this schedule will be addressed in your confirmation packet. Early arrivals to a camp location are allowed but must be handled through the Starmakers office for early arrival fees and specifics.

Cost: Camp costs are listed by clinic. The prices include instruction, room, and board. Commuter students receive all instruction but no housing or meals for the price listed. All directors, assistant directors, parents or chaperones, and managers will pay the director's fee listed. Key deposits will be required at each campus; because fees vary from campus to campus, this will be addressed in your confirmation packet. Note: Your registration will be processed faster and more efficiently if you send only one (1) check per team rather than separate checks for individual team members. If individual checks are sent, please be informed that directors will be asked to help collect insufficient fund checks plus the surcharge fee. Remember, full payment must be received prior to your arrival at camp.

Refunds: The $50 deposit is nonrefundable.  In case of cancellation, with at least a two weeks notice prior to the start of camp, the participant may receive a refund less the $50 deposit.  This refund will be mailed from the Starmakers' office after camp.

Medical Staff: Medical Staff will be present at each camp to be available as needed to directors and dancers.

Medical Permission Form: All participants will be required to bring a signed medical form to camp. This will be found in your confirmation packet. Directors will collect these prior to camp and bring them to registration.

Registration: Complete the registration form and return with payment to Starmakers Dance & Performance Camps, Inc. Upon receipt of your registration in our office, we will send you a confirmation packet with camp specifics. Registration must be received 14 days prior to Day 1 of camp; any registrations received after that day are subject to $75.00 team late fee. To print a copy of our registration form on your printer and register by mail, you will need Adobe Acrobat Reader.
